Teladoc Health is seeking a highly motivated Physician Case Manager (PCM) to play a vital role in remotely coordinating and managing patient care. This position leverages telecommunication technology to provide virtual consultations, collaborate with healthcare teams, and ensure seamless delivery of patient-centered care. You will serve as a liaison between patients, providers, and other stakeholders, ensuring effective communication, care coordination, and optimal health outcomes.
Act as the primary point of contact for patients to understand their medical concerns, deliver expert guidance and foster clear communication and collaboration.
Manage and follow up on cases referred by Teladoc Health.
Conduct comprehensive summary of patients medical conditions, including reviewing medical history, symptoms, and treatment plans.
Collaborate with patients to develop personalized care plans tailored to their unique needs, preferences, and available resources.
Provide clear explanations of Expert Medical Opinion (EMO) reports to patients.
Continuously monitor and evaluate patients progress, adjusting care plans as needed and offering appropriate medical guidance and education.
Complete all tasks within designated turnaround times.
Partner with the Medical Director and Care Coordinator on quality and service improvement initiatives.
Deliver remote advisory services including (but not limited to) EMO, health advisories, chronic disease care, coaching/education, and referrals.
Education:
Required: Medical degree from an accredited university/institution
> 7 years experience post medical degree
Desirable: Higher diploma/Masters in Family Medicine or other relevant specialty
Licenses:
Required: Valid medical practice license issued by Indonesian Medical Council (KKI)
Languages:
Proficient in Bahasa Indonesia and English
Computer Skills:
Proficient in using telecommunication technology and telemedicine platforms for virtual patient consultations
Soft Skills:
Excellent communication and interpersonal skills to build rapport with patients and collaborate effectively with remote healthcare teams.
